PROSPECTIVE IMMIGRANT QUESTIONNAIRE

Please answer the following questions in as much detail as possible. Some questions will not be relevant to you. If the question is not relevant please answer "N/A". Please answer the questions in English. It is very important that you complete question 20 on this Questionnaire so that we can communicate with you very promptly. The following questions should be answered by the "principal applicant" but please also refer to section 21 of the Questionnaire.
